What Will Happen On My First Visit?

Please assist us at the time of your initial visit to the office by:

• Answering a COVID-19 questionnaire that our staff will provide you with at time of checking in. Your temperature will be taken and recorded at every visit.
• Bringing your referral slip and X-rays from your referring dentist.
• Listing down the medications you are currently taking. 
• Bringing any forms or insurance cards with you, if you have dental insurance, to the appointment.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Notify the office if you have a medical condition or concern prior to a dental or periodontal procedure and surgery (e.g., artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, or hypertension, cancer treatment or taking blood thinners of any kind).
